"The Most Sadistic" is a song by American rapper Necro, featuring his brother, Ill Bill, which was released in 1999. It was the first and only single from his debut album, I Need Drugs, released the following year.

The song was officially remixed once since its initial release, in 2003 for Rare Demos and Freestyles Volume 1.
