The Macedonian First League (Macedonian: Македонска Прва Лига - Makedonska Prva Liga, or just Prva Liga) is the highest professional football competition in the Republic of Macedonia.

The competition is currently contested by 12 clubs, playing each other 3 times over 33 rounds. The winner of the league enters into the qualifying rounds of the Champions League, while the second and third placed teams are entered into the qualifying rounds of the UEFA Cup along with the winner of the Macedonian Cup. At the end of the season, the bottom 2 teams are relegated to the Macedonian Second League while the 10th and 9th placed teams play a play-off with the 3rd and 4th placed teams of the Macedonian Second League.

Prior to 1992, Macedonian clubs competed in Yugoslavian league. Since the season 1992/93 Macedonian club compete in their own national league. A total of 34 have entered the First League so far. Below is a list of First League clubs who have competed in the competition since 1992;
