I reverted your italicising because it is not usual practice to italicise the teasers. This is because sometimes in the synopses the teasers are summarised together with events in the main text. The italics in Army of Ghosts are put there because of narrative reasons, i.e. a framing sequence of sorts that places it outside the narrative structure of the episode: examples of framing sequences treated in this way are in Father's Day and the four The Trial of a Time Lord serials. Love & Monsters did not have this for practical reasons: Elton's scenes were spliced together with the rest of the narration and to switch between the various cuts would have been too confusing and render it less readable.

How do we know the visions are "created by" Bilis? It seems to me that this is a consequence of the weakening of the time rift rather than anything Bilis has done, and in particular, he didn't specifically recreate the appearance of Diane.
