This is an album cover for Yuragu Sora（ユラグソラ）, by Japanese singer Haruka Shimotsuki.

Source: http://largo.cside.to/yuragu/ (album's official site) (I have the scan myself, but I scanned it at my friend's house and it turned out worse than the album cover Shimotsuki had on her website so I thought I might as well pick the better one).
