Route 216 takes the following route through the State:
- Charlestown: 2.5 miles; U.S. Route 1 to Westerly city line
- Ross Hill Road
- Westerly: 2.1 miles; Charlestown town line to Hopkinton town line
- Ross Hill Road, Church Street, South Main Street and North Main Street
- Hopkinton: 3.5 miles; Westerly city line to Connecticut State line at Route 216
- Alton-Bradford Road, Ashaway Road, Main Street and High Street
