Directly under it, kindly state your reasons for wanting nothing, as well as what you'll do with it once you have it. Don't forget to add your signature, so that we'll know who to not give anything to, should your request be approved.

Hello, I would like a man, a plan, a canoe, pasta, hero's rajahs, a coloratura, maps, snipe, percale, macaroni, a gag, a banana bag, a tan, a tag, a banana bag again, or a camel, a crepe, pins, spam, a rut, a Rolo, cash, a jar, sore hats, a peon, a canal, Panama. I need them because I've run out of booze.
