Her role in the 2003 British film AfterLife, directed by Alison Peebles, brought her a BAFTA Scotland award for best first time performance and Best Actress in the Bratislava International Film Festival, 2004 [2]. A Channel 4 review said:

Debut actress Paula Sage holds her own as the feisty Roberta opposite screen veteran Lindsay Duncan, and talented younger actors like Kevin McKidd and Shirley Henderson. If anything holds back Sage's acting career, it will be the shortage of roles available to actors with Down's Syndrome

Afterlife won the Audience Award at The Edinburgh Film Festival 2003. It also won Sage a role as Donna McCabe in BBC Scotland's River City soap.
