I am searching and trying to find out if my late husband, Dale A. Livingston's, insurance policy 74500583 with National Life and Accident Insurance Company, is valid. I have a copy of the application and the policy. There are no numbers or addresses for contact purposes.

My husband died of a tragic accident 5 years ago 10/21/01. I have just located this policy and would like any information you can provide me.

I have an insurance policy for my late wife Vivian Z. Tucker, insurance policy number 48 51 420 031 with National Life and Accident Insurance Company, could you tell me if this is a valid policy. There was no number or address for contact purposes. I have a copy of the insurance policy.

My wife died June 24, 2006.

I have an isnurance policy with The National Life and Accident Insurance Company. The insurance policy number is 2662888. It was taken out on my life when I was age 9 years old.
