Where, and how is it revealed that his name is "Millions Knives", rather than simply "Knives"?

It appears in the manga once or twice; ie. in the last chapter where the army guys say "Release both Millions Knives and Vash the Stampede to us at once!" --Gwern (contribs) 15:41 3 May 2008 (GMT)
