Welcome to the Wikipedia, Kristenq! And thanks for fixing the Wikilink over on the Adverse effect (medicine) article. Hope you enjoy editing here and becoming a Wikipedian! Here are a few perfunctory tips to hasten your acculturation into the Wikipedia experience:

The best way for you to do a survey is to use a persons email address as the unique key. While this does not stop people from entering false information or duplicate information, you can assess each entry individually to determine if the data appears valid. I think its a case of trusting that the community as a whole has no purpose to vandalise such statistics. Enigmatical 00:17, 24 April 2006 (UTC)
