[edit] Primary meaning

There are ample examples here of the meaning knocked unconscious with a single hit, typically from behind the head, but no examples that clearly mean "designed to cause as much damage as possible", "usually a punch to the face". When the phrase was first introduced to me (1991, Melbourne), it was as a hard hit to the back of the head to knock the victim unconscious. I'm inclined to modify the article to put this as the primary meaning. I've just googled for the phrase, here are a couple of instances of people discussing its meaning. (Each paragraph is from a different comment.) :
