http://knihovna.nkp.cz/Nkkr9904/9904204.html (a link from the Czech-language Wikipedia) refers to a monograph on the composer in Czech. This should be followed up; if the monograph/PhD dissertation/... has any confirming evidence (convincing photo of birth certificate, for example) then this is what one looks for, or close enough to it.
