Interestingly, I came across this page while looking for something else. I did read David Andrews book many years ago, and thought it an important contribution to knowledge.

This article is very poorly written, in terms of the English usage and other factors, so I intend to do something about it.

To do list:
List / decide Sections and Sub-Sections
Re-write paragraphs to improve clarity and accuracy
Make citations and references correct
