S.R. 120 Alternate was created in 2007 after S.R. 120 Loop was decommissioned and S.R. 120 mainline was rerouted on the southern portion of the loop. As a result. S.R. 120 Alternate was created to take over the northern portion of the loop. Both intersections of old mainline S.R. 120.
A portion of S.R. 5 runs multiplexed with S.R. 120 Alternate from US 41 west to S.R. 120 mainline.
