Moghedien was alive in book 11 as herself, not ressurected, so she cant have died in book 9.
Asmodean, deceased (killed by Graendal. This is confirmed in an interview with Robert Jordan.) This is not true. He has never confirmed in anyway that Graendal was the killer.
