[edit] How much do non-driver ID cards cost? Application procedure?

In conjunction with the current voting rights case (Indiana law requiring photo ID for voting) it would be useful to add info on how much non-driver ID cards cost and what the procedure is for obtaining one in the several states. If someone has that info please add it.
