Paxton Fettel, a telepathic commander controlling a battalion of clone supersoldiers, has escaped from one of ATC’s (Armacham Technology Corporation) facilities. He uses his psychic abilities to command the clones into assaulting ATC headquarters as well as the South River Wastewater Treatment Plant.

The First Encounter Assault Recon (F.E.A.R.) team is called in to assess the situation. Commissioner Betters briefs the new point man (the protagonist), and veterans Jankowski and Jin Sun-Kwon, about Fettel and their mission: to take down Fettel to sever the mind link between him and the soldiers who will shut down once they no longer have an objective.

The player and Jankowski head for the abandoned Auburn district after the tracking device implanted in Fettel’s head leads them there. They discover the body of an ATC employee, Chuck Habeggar. Fettel disappears and Jankowski and the protagonist are diverted to the South River Wastewater Treatment Plant to evaluate the situation occurring there. Fettel is shown to be at the plant as well. Shortly after their arrival, the Delta Force entourage is killed and Jankowski disappears. The player makes his way through the plant, uncovering ATC activity and encountering soldiers along the way. The soldiers, along with Fettel, withdraw from the plant and head to ATC headquarters with the protagonist on their heels.
