Copy and paste this into your article. If there is no data for a particular section just leave it blank. For BDO and PDC input the years they played in each particular organisation (where applicable) eg BDO = 1987 to 1993.

For each tournament add the best performance eg | Matchplay = QF 1998 or | World Masters = Runner-up 1952. If they haven't played in a particular or info is not available just leave blank.

For multiple tournament wins list all the tournaments first, then the years in the same order (as in this example):

The HTML markup produced by this template includes an hCard microformat, which makes the person's details parsable by computers, either acting automatically to catalogue articles across Wikipedia or via a browser tool operated by a reader, to (for example) add the subject to an address book. Date-of-birth ("bday") parameters will only be included in the microformat if {{Birth date}} or {{Birth date and age}} are used in the infobox.
