[edit] Missed Penalty Kick

"Massaro missed a penalty kick against Brazil in the 1994 World Cup which cost Italy the championship."

It is perhaps noteworthy that on Massaro's PK, the Brazilian goalkeeper came off his line early (approached the spot where the PK was taken) which helped him make the save. Therefore, it is perhaps unfair to put the blame emtirely on Massaro for the miss.
