Highway 21 is a north-south state highway in north central and west central Arkansas. Its northern terminus is at the Missouri state line in Blue Eye where it continues north as Route 13. Its southern terminus is officially at U.S. Highway 64 just east of Clarksville, though northbound it is posted as beginning at Interstate 40 and runs north along Highway 103 for 3 miles then runs east along US 64 for 1 mile before turning north.
With the exception of a short 1/2 mile concurrency with U.S. Highway 62 at Berryville Highway 21 is a two-lane rural highway.
