I disagree that the barwoman scene is in his head. At several other points in the film he tells people that irritate him that he is going to kill them, the laundrette scene for example where the woman does react and other people are present and also react. Why would the bar scene be the only one where it's in his head? As for the woman giving no reaction, I've been a barman myself in the past in a few rough joints and you are trained not to respond to any threats directed at you as it is thought responding can make the situation worse.
I believe that the woman does show a reaction to Batemans threat as her facial reaction shows, even though she says nothing.
