United States Oath of Allegiance form for James Hubert Blake, completed in 1920. The Oath of Allegiance permits Eubie and his wife, "Aves Elizabeth Cecelia Blake," to travel to England and France in August 1920 for theatrical engagements. Page 2 includes the Affidavit of Identifying Witness, Noble Lee Sissle, who has known the applicant (Eubie) for six years.
